目的 研究结直肠腺瘤发生相关的危险因素,为结直肠早癌病变的筛查及诊疗提供预警信息。方法 选取2014年1月至2020年6月于河北医科大学第二医院行结肠镜检查经病理诊断为结直肠腺瘤的患者1 126例作为病例组,以同期行结肠镜检查结果阴性的患者1 800例作为对照组。收集患者性别、年龄、吸烟史、饮酒史、排便习惯(正常、腹泻、便秘、腹泻与便秘交替)、总胆固醇及甘油三酯水平等资料。进行多因素Logistic回归分析,探究上述因素对结直肠腺瘤发生的影响。为了进一步排除混杂因素的干扰,组间1∶1匹配,进行多因素条件Logistic回归分析,探究结直肠腺瘤发生的独立危险因素。结果 多因素Logistic回归分析显示,性别、年龄、饮酒史、吸烟史、排便习惯、总胆固醇和甘油三酯水平为结直肠腺瘤发生的独立危险因素(P<0.05)。多因素条件Logistic回归分析显示,排便习惯及甘油三酯水平是结直肠腺癌发生的独立危险因素,慢性腹泻、慢性便秘患者发生结直肠腺瘤的风险高于排便习惯正常者(P<0.05),甘油三酯水平较高患者发生结直肠腺瘤的风险高于甘油三酯水平正常者(P<0.05)。结论 排便习惯及甘油三酯水平是结直肠腺瘤发生的独立危险因素,慢性腹泻、慢性便秘、甘油三酯升高者发生结直肠腺瘤风险更高。  相似文献

Background: Sorafenib, an oral multi-kinase inhibitor of rapidly accelerated fibrosarcoma; vascular endothelial growth factor receptor-2/3, platelet-derived growth factor receptor, c-Kit, and Flt-3 signaling, is approved for treatment of advanced hepatocellular carcinoma (HCC). However, the benefit of sorafenib is often diminished because of acquired resistance through the reactivation of ERK signaling in sorafenib-resistant HCC cells. In this work, we investigated whether adding LY3214996, a selective ERK1/2 inhibitor, to sorafenib would increase the anti-tumor effectiveness of sorafenib to HCC cells.Methods: The Huh7 cell line was used as a cell model for treatment with sorafenib, LY3214996, and their combination. Phosphorylation of the key kinases in the Ras/Raf/MAPK and PI3K/Akt pathways, protein expression of the cell cycle, and apoptosis migration were assessed with western blot. MTT and colony-formation assays were used to evaluate cell proliferation. Wound-healing assay was used to assess cell migration. Cell cycle and apoptosis analyses were conducted with flow cytometry.Results: LY3214996 decreased phosphorylation of the Ras/Raf/MAPK and PI3K/Akt pathways, including p-c-Raf, p-P90RSK, p-S6K and p-eIF4EBP1 activated by sorafenib, despite increased p-ERK1/2 levels. LY3214996 increased the anti-proliferation, anti-migration, cell-cycle progression, and pro-apoptotic effects of sorafenib on Huh7R cells.Conclusions: Reactivation of ERK1/2 appears to be a molecular mechanism of acquired resistance of HCC to sorafenib. LY3214996 combined with sorafenib enhanced the anti-tumor effects of sorafenib in HCC. These findings form a theoretical basis for trial of LY3214996 combined with sorafenib as second-line treatment of sorafenib-resistant in advanced HCC.  相似文献

The freeze–thaw resistant performance of a tunnel fireproof coating (TFC) has an important impact on bonding property and durability. The influence of redispersible emulsion powder, polypropylene fiber and air-entraining agent on TFCs was studied. Transverse fundamental frequency and ultrasonic sound velocity were used to evaluate the damage degree of TFC, and the mechanism was revealed by SEM and pore structure. The results show that the most beneficial effect on bond strength of TFC is redispersible emulsion powder, followed by air-entraining agent, and then polypropylene fiber. After freeze–thaw cycles, the cumulative pore volume of micropores in the TFC increases obviously, while the porosity of macropores does not change significantly. A prediction model was proposed, which can calculate the bond strength from the damage degree of TFC under freeze–thaw cycles. The achievement can promote the application of TFC in cold regions.  相似文献

The purpose of this study was to evaluate the role of 2-D speckle tracking imaging in assessing left ventricular diastolic function in patients with connective tissue disease (CTD). A total of 98 CTD patients and 32 healthy controls were prospectively recruited. Early (E) and late (A) diastolic velocities of the transmitral flow were measured by pulsed Doppler echocardiography. Peak early diastolic myocardial velocity (E') was calculated on tissue Doppler echocardiography. The longitudinal strain rate (SR) was calculated as the average of three apical views, while circumferential and radial SRs were measured in three short-axis views. Pulmonary arterial hypertension (PAH) was defined as systolic pulmonary arterial pressure (sPAP) >36 mm Hg. Compared with the control group, CTD patients exhibited significant impairment of left ventricular diastolic function, manifested as lower global SR during early diastole (SRe) in the longitudinal deformation and higher E/SRe in both longitudinal and radial deformation. CTD-PAH patients had significantly lower SRe and higher E/SRe values in both the longitudinal and radial deformation compared with the patients with CTD without PAH. Pearson's correlation analysis revealed that sPAP levels correlated positively with E/E′, longitudinal E/SRe, circumferential E/SRe and radial SRe, and it correlated negatively with septal E′ and radial E/SRe. Receiver operating characteristic curve analysis suggested that E/E′, longitudinal E/SRe and radial SRe could be used to predict PAH. The present study indicates that 2-D speckle tracking imaging is a useful method for evaluation of left ventricular diastolic function, and these derived parameters can serve as good predictors of PAH, but it may not be superior to the commonly used E/E′ in CTD patients.  相似文献
